1. Describe the domains of Social Determinants of Health and how these factors influence health outcomes for a population. 2. Explain the four parts of Medicare and coverage of each entity that affect patient care, placement, and copays.   3. Outline the process for identifying patients who are appropriate for care coordination in the acute care setting, across the life span. 4. Define the role of the Social Work Case Manager in coordinating care specific to a disease process and/or chronic condition.   5. Identify the process for treatment & coordination of care that incorporates goals of care decisions, and guardianship/conservatorship requirements.

TIFFANY FERGUSON, LMSW, CMAC, ACM

Tiffany Ferguson is CEO of Phoenix Medical Management, Inc. Tiffany serves as an adjunct professor at Northern Arizona University, Department of Social Work, and on the American College of Physician Advisors (ACPA) Observation Committee. Tiffany is co-author of The Hospital Guide to Contemporary Case Management through HcPro. She is a contributor for RACmonitor and Case Management Monthly; she also serves on the editorial board for CMSAToday and Care Management. She is a weekly correspondent on SDoH for the news podcast Talk Ten Tuesday. After practicing as a hospital social worker, she went on to serve as Director of Case Management and quickly assumed responsibilities in system level leadership roles in Health & Care Management, which includes CM, UR, CDI, HIM, and coding. She has held C-level responsibility for a large employed medical group which included value-based arrangements and outpatient care management. Tiffany is a graduate of Northern Arizona University and received her MSW at UCLA. She is a licensed social worker, ACM, and CMAC certified.

MARIE STINEBUCK, MBA, MSN, ACM

Marie Stinebuck is the Chief Operating Officer (COO) of Phoenix Medical Management. With nearly 30 years of nursing experience, Marie has spent nearly 20 years specializing in case management. Her career has been primarily focused on the acute hospital setting, including the implementation of outpatient case management programs. Marie has held executive leadership roles overseeing case management, utilization review, denials prevention, clinical documentation improvement (CDI), and medical record integrity. She is also the author of numerous articles related to case management, utilization review, and denials prevention. Her true passion is education, and she has been instrumental in expanding Phoenix Medical Management’s education program. Marie has served on the Arizona Board of the American Case Management Association (ACMA) and is certified through the ACMA. She holds an MBA and a Master of Science in Nursing (MSN) with a focus on Nursing Leadership.

KALIE WOLFINGER

Kalie Wolfinger, LCSW, brings nearly 13 years of clinical social work expertise across primary care and hospital settings. Her diverse experience includes roles in emergency department clinical social work, integrated behavioral health within primary care, and case management in hospital inpatient psychiatric units and general hospital settings. In addition to her healthcare contributions, Kalie is an educator in higher education, teaching both Bachelor of Social Work and Master of Social Work programs. She also maintains a private practice, specializing in supporting first responders, crisis intervention, trauma, and dissociation.

Week 1: One hour class reviewing the history and roles of social work in case management.

Week 2: One hour class understanding government and commercial payers.

Week 3: One hour class discussing Conditions of Participation specific to discharge planning standards.

Week 4: One hour class discussing diagnosis-specific care coordination needs for populations.

Week 5: One hour class detailing goals of care and patient rights.

Week 6: One hour class detailing social worker hospital practice settings.

Course Completion & CE Information

Course completion requirements: Attendees must attend the entire course and complete a course evaluation to be eligible for CE credit. Certificates of completion will be emailed within 10 business days of course and evaluation completion. Phoenix Medical Management [provider number pending approval] is an ACE provider to offer social work continuing education by the Association of Social Work Boards (ASWB) Approved Continuing Education (ACE) program. Regulatory boards are the final authority on courses accepted for continuing education credit. ACE provider period: [dates pending approval].
